Photo Consent
I agree to allow Food Bank of Northern Nevada unrestricted use of photographs taken of me and/or my child in the course of participation in activities sponsored by Food Bank of Northern Nevada or a partner agency of the Food Bank of Northern Nevada.
I understand that Food Bank of Northern Nevada intends to use such photograps only in connections with official Food Bank of Northern Nevada publications and documents.
Statement of Confidentiality
The Food Bank of Northern Nevada requires that strict confidentiality be maintained with respect to all information obtained by volunteers concerning the organization, as well as clients and others they serve. The volunteer shall not disclose any information obtained in the course of his/her volunteer placement to any third party without prior written consent from Food Bank of Northern Nevada. This includes but is not limited to information pertaining to financial status and operations such as budget information, donations of money or gifts in kind, donor information, salary information and information pertaining to clients, staff or other volunteers.
Safety Agreement
I understand that I am volunteering my services to the Food Bank of Northern Nevada. I warrant that I am competent to perform the services I am offering the Food Bank, and I agree to perform these services in a safe manner. I understand that the Food Bank is authorized to direct and coordinate my efforts with those of other volunteers. I agree to follow the directions given by Food Bank personnel. ask for help as needed and report possible hazards and unsafe activities to staff and/or volunteer leads.
Signing in and out is a requirement for all volunteers. By signing in, volunteers agree they will abide by all safety regulations posted and will follow directions from staff and volunteer leads.
Release of Liability
As consideration for volunteering for Food Bank of Northern Nevada, I hereby agree that I, and my assignees, heirs, guardians, and legal representatives, will not make a claim against or sue Food Bank of Northern Nevada or its employees, agents, or contractors for injury or damage resulting from the negligence, whether active or passive, or other acts, however caused, by any of its officers, employees, agents, or contractors of Food Bank of Northern Nevada as a result of my volunteering. I HEREBY RELEASE AND DISCHARGE FOOD BANK OF NORTHERN NEVADA AND ITS OFFICERS, EMPLOYEES, AGENTS, AND CONTRACTORS FROM ALL ACTIONS, CLAIMS, OR DEMANDS THAT I, MY HEIRS, GUARDIANS, AND LEGAL REPRESENTATIVES NOW HAVE, OR MAY HAVE IN THE FUTURE, FOR INJURY OR DAMAGE RESULTING FROM MY PARTICIPATION IN THE PROJECT.
